Please call (209) 726-3000 during normal business hours to schedule this service.

Dr. Anderson is a fully trained and qualified DOT Certified Medical Examiner. He is able to efficiently and competently examine the health of each commercial driver's license candidate to determine if they are fit for safe driving on public roads.

Please bring along any supporting medical documentation if you have any complicating factors in your recent health history. This includes:

• Corrective Lenses (with completed Vision Evaluation Report forms if applicable)
• Hearing Aides
• List of actively prescribed drugs
• Medical waivers / exemptions
• Skill Performance Evaluation (SPE) Certificates
• Latest blood lab results
• Completed Insulin-Treated Diabetes Mellitus Assessment Form

You will walk away from this appointment with the completed Medical Examiner's Certificate (also updated in the National DOT database) and the Medical Examination Report to take back to the DMV.
